SBA Paycheck Protection Program Forgiveness

The Small Business Administration is now ready to receive forgiveness applications for first or second draw loans obtained during Round 3 of the Paycheck Protection Program.  The following forgiveness applications can be completed and dropped by your local office along with any required documentation needed to submit the application to the Small Business Administration.  We will then process your forgiveness application and submit the application and any required documentation to the SBA for forgiveness.

 

Form 3508 S -- Use this form for any first or second draw loan with an original loan amount under $150,000.

Form 3508EZ -- Use this form for any first or second draw loan with an original loan amount over $150,000 if you:

  • Did not reduce annual salary or hourly wages of any employee by more than 25% during the Covered Period compared to the most recent full quarter before the Covered Period; and
  • Did not reduce the number of employees or the average paid hours of employees between January 1, 2020 and the end of the Covered Period; or
  • The Borrower did not reduce annual salary or hourly wages of any employee by more than 25% during the Covered Period compared to the most recent full quarter before the Covered Period; and
  • Was unable to operate during the Covered period at the same level of business activity as before February  15, 2020 due to compliance with requirements established by governmental authorities related to maintenance of sanitation standards, social distancing or any other work or customer safety requirement.
 

 

Form 3508 -- Use this form for any first or second draw loan that cannot be reported on Form 3508S or 3508EZ.
